Abstract


PT. Arista Semesta Alam (ASA) is an agricultural pharmaceutical distributor and rice seed producer located in Madiun Regency, Indonesia. The company's current occupational health and safety (OHS) practices are suboptimal, leading to workplace accidents. Interviews with workers revealed that common accidents on the production floor include lacerations, shortness of breath, skin irritation, and fractures. Workers also expressed concerns about the lack of hazard control measures, insufficient personal protective equipment (PPE), and unclear work
instructions. In response to these accidents and concerns, the company conducted a hazard identification, risk assessment, and risk control (HIRARC) study to identify potential hazards. The risk assessment identified 1 high-risk (red) hazard, 25 medium-risk (yellow) hazards, and 6 low-risk (green) hazards. Control measures were proposed, including the creation of forkliftlanes, the implementation of warning signs (e.g., "No riding on forklifts"), clear work instructions for lifting heavy objects, the provision of PPE (e.g., safety boots, safety vests), and the supply of dust masks. The proposed control measures were shown to have a positive impact, as the post-control risk assessment revealed a reduction of 7 medium-risk (yellow) hazards and 25 low-risk (green)
hazards.
